We are now using an env_ prefix for environment functions. Rename these
commonly used functions, for consistency. Also add function comments in

lzmadec: Use the same type as the lzma call

With sandbox on 32-bit the size_t type can be a little inconsistent. Use
the same type as the caller expects to avoid a compiler warning.

Remove the cmd_ prefix from command files

Now that they are in their own directory, we can remove this prefix.
This makes it easier to find a file since the prefix does not get in the
way.
