vsprintf:fix: Change type returned by ustrtoul
The ustrtoul shall convert string defined size (e.g. 1GiB) to unsigned long type (as its name implies).
Up till now it had returned int, which might cause problems with large numbers (GiB range), when interpreted as U2 signed numbers.

lib/vsprintf.c: don't special-case pointers to address null
The %p format of printf() would print a pointer to address null as "(null)". This makes sense in a real OS where a NULL pointer must never be dereferenced, but this is a bootloader, and there are cases where accessing the data at address null makes perfect sense.
Remove the special case in lib/vsprintf.c using "#if 0" with a comment to make clear this was an intentional change and to stop re-adding this code.

Add safe vsnprintf and snprintf library functions
From: Sonny Rao <sonnyrao@chromium.org>
These functions are useful in U-Boot because they allow a graceful failure rather than an unpredictable stack overflow when printf() buffers are exceeded.
Mostly copied from the Linux kernel. I copied vscnprintf and scnprintf so we can change printf and vprintf to use the safe implementation but still return the correct values.

Add assert() for debug assertions
assert() is like BUG_ON() but compiles to nothing unless DEBUG is defined. This is useful when a condition is an error but a board reset is unlikely to fix it, so it is better to soldier on in hope. Assertion failures should be caught during development/test.
It turns out that assert() is defined separately in a few places in U-Boot with various meanings. This patch cleans up some of these.
Build errors exposed by this change (and defining DEBUG) are also fixed in this patch.

Make sure that argv[] argument pointers are not modified.
The hush shell dynamically allocates (and re-allocates) memory for the argument strings in the "char *argv[]" argument vector passed to commands. Any code that modifies these pointers will cause serious corruption of the malloc data structures and crash U-Boot, so make sure the compiler can check that no such modifications are being done by changing the code into "char * const argv[]".
This modification is the result of debugging a strange crash caused after adding a new command, which used the following argument processing code which has been working perfectly fine in all Unix systems since version 6 - but not so in U-Boot:
int main (int argc, char **argv) { while (--argc > 0 && **++argv == '-') { /* ====> */ while (*++*argv) { switch (**argv) { case 'd': debug++; break; ... default: usage (); } } } ... }
The line marked "====>" will corrupt the malloc data structures and usually cause U-Boot to crash when the next command gets executed by the shell. With the modification, the compiler will prevent this with an error: increment of read-only location '*argv'
N.B.: The code above can be trivially rewritten like this:
while (--argc > 0 && **++argv == '-') { char *arg = *argv; while (*++arg) { switch (*arg) { ...
